Understanding FinalForms: Answers to Your Most Common Questions
Every year we have countless people with questions about FinalForms. Hopefully by answering some of the most frequently asked questions, we can clear a few things up!
What is FinalForms and why do we have to use it?
FinalForms is an online system for compliance. Hundreds of schools use it to help with collecting documents and ensuring that students and parents are complying with school policies. It is also used by coaches to communicate with students and parents about practices, games, and meetings.
What is the link for FinalForms?
You can also go to FInalForms.com and hit the "Login" button. It will take you to a screen asking for your school district's name. Our name is "Warren Township Metropolitan Schools."
How do I create an account?
Parents must create an account first before students will have access. To do this:
Click on the link above then locate the parent icon and click "NEW ACCOUNT"
This will take you to a registration page. Fill out your name, date of birth, and email then click "REGISTER"
Afterwards, you will receive an email asking you to confirm your registration. If you cannot find it, check your Spam or Junk folder.
Next, create your password then click "Confirm Account"
Congrats! The parent account is created! Now you can register your first child.
To Create a Student Accounts:
Parents login to their account and click "ADD STUDENT"
Type in the student's legal name along with other required information. Click "CREATE STUDENT"
If your student is planning on participating in any sport, click the appropriate checkbox. Click "UPDATE" after making your selection(s). These selections can be changed until the registration deadline!
Complete each form and sign your full name in the parent signature field
If you have more than one student to register, click "MY STUDENTS" then repeat steps above.
Once the student account is created, they should get an email to confirm the account and to sign their forms.
How do I upload a document/my physical?
On the parent account, find the Folder Icon under "Actions"
Click on "Choose File"
Find the file you are trying to upload and click open.
The file should be uploaded to your student's FinalForms Account!
Why do I need to fill out these forms?
These forms include items such as medical history, contact information, consent for medical care, Warrior Acknowledgement, and more. Each form has its own purpose and is required for participation in any Warren Sports.
My coach said I'm red on FinalForms and/or that I can't practice until I am green on FinalForms. What does that mean?
In order to participate in sports here at Warren, you must have a valid physical uploaded along with signatures by both the student and the parent/guardian.
Additionally, if there is a note on your physical or you have been under the care of a doctor for an injury, you may have additional clearance requirements. In this case, you should receive an email stating reasons behind new participation restrictions.
Why was my physical rejected?
There are many reasons that a physical could be rejected. Make sure all of the points below are correct before uploading your physical (page 3 of IHSAA packet)
Name and date of birth are written at the top of the document
The physical is completed on or after April 1 of the new school year (2025-2026 school year needs physicals completed on or after April 1, 2025).
A clearance box has been checked
The Healthcare Provider section at the bottom is completely filled out -- Name of the provider, date of examination, address, phone number, license number, and signature.
The image uploaded is clear and visible
No whiteout marks
